I did both the sides and bottom. I have also filled in the gaps on the normally exposed foam mid-sole so that the entire sole is harder and will stand up to wearing.
Yes you can leave the hard shoe form in there. The reason I take it out is that it helps with my feet fitting in there and it makes it SO much easier to install the wires and battery.
