It would be worth, instead of posting a question asking for it, to simply just google for it. I did that very thing and found a fairly reasonable sized one in a matter of seconds. I'm not being mean or trying to come across as rude, but these type of questions bog down the forum, as we need to keep it open to mainly props and their making.
And now, with that said, the teacher in me is not going to give you the location of the label I found, as you must learn for yourself

This is my first post, so forgive me if this doesn't quite go correctly. I'd like to contribute a file for a Ministry of Magic ID. Hopefully it is useful. It is set up to print the whole thing on an 11x17 page (the PDF file).
I used the font Kingthings typewriter 2 in a rust or maroon color to fill in personal information. After printing, fold in half on the blue dashed line and glue or adhere in whatever way you like the sides together to create a double sided document, then cut or fold on the magenta lines as marked. Fill in signature with a green fine point sharpie and take your fingerprints on the right panel. Staple the 2 pieces together on the fold. I've included a shot of an assembled ID so hopefully it is easy enough to see how it is put together.
