If your talking about the cover, you can get them on ebay. I think that one was a extra with the dvd.

I made a half assed version of this a few years ago and I basically sculpted the face in clay on top of a piece of folded card board that was cut to the size a wanted the "book" to be. Then I used a few layers of liquid latex to cover the whole thing. I airbrushed the paint, added a notebook inside and I was done. I didnt want to use any art inside as I was giving this as a gift to someone who would actually use the thing as a notebook. LOL
