What Battlestar from what colony

There were 12 origianal battlestars. One for each colony. Does anyone have a list of names (Battlestar & what colony)?

The only one that I know of is Galactica / Caprica.

In total, there are only seven Battlestars I would consider canon. Debate is welcome.

These five were at the Peace Conference:

This one destroyed prior to conference:

For the RIS BSG: There are two Columbias. The first one was destroyed in a battle over the ice planet in the "Razor Flashback" webisodes (and also seen in "Razor"). Sometime after that, a second Columbia was constructed and later destroyed by the Cylons during the events in the Mini-Series (as Adama lists the Columbia amongst the ships that were destroyed).
