Probably paint a light color on the book. do a graphite trace over like most of the folks do for the U2 journal then use a pointed stylus to engrave the design.
I found (after destroying one book) that if you have a pointed tool you can press and drag it without damaging the surface to imprint what you want.
For the lines on the edges I marked off where I wanted them with a red Sharpie fine tip marker (it does show up on the black) and then I used a straight edge and pointed carving tool, dragging it along with enough pressure to indent the groove.
Also if you have a tool with a curved tip you can go inside on the spine and add the raised lines.
