What are the dimensions of the passport? Would you say that the cover is leather?
I used standard passport dimensions of approximately 87mm x 124mm, and I'd kinda assumed the cover was card... although I could be wrong on this.
I kinda looks to be a soft leather to me... Mh, it would be extra trouble I guess haha... I want to give it a shot ^^

The cover is definately cardstock. Here's a higher resolution image of the passport and an accurate but badly photoshopped (by me) image of Agatha.
Aw, that's much higher res than the image I'd been working with, I could have done a far more accurate job with that...

How many pages and what would the order be? Do we get to see this on the movie? I've been skipping some scenes, but not sure if we got to see it...

I'm afraid you'd have to do a little research on historical passports to check out how many pages were usually in these kind of documents. It shouldn't be too much of a stretch. Propnomicon had a nice post about passports the other day.
With the help of that higher resolution image I'll have a slightly more accurate version of the passport at some point in the next day or two. I've completed the background pattern, it's just tricky nailing which typefaces have been used. Think I've scrolled through enough germanic fonts for one day.
Front cover won't change.

Found another pic of the passport online and also the Hotel menu (made by graphic designer Annie Atkins)

Here are the updated backgrounds for the passport pages. Page size is assumed to be 87mm x 124mm, and I've provided approximately 3mm bleed on each edge of the images so you can trim them down. Think the only way I could get this more accurate would be if I could get my hands on the real thing.

If anyone's up for vectorizing (thought I might do it myself but there's other stuff I need to work on. Like the BTTF jacket lining.:lol) then here's the front cover of the book that I cleaned up a bit plus screencaps for the front, back and side. Also photoshopped the authors image:
