This pic shows the orange brush tucked in. Since you can see the cap on and there is extra space, I think it's safe to say one of the little brushes was attached,, especially since it's of no use without it.
Now to determine if it is the small or large brush.

So studying the new pics above they are large enough to clearly see the writing and they match the Butler Proxabrush I posted earlier that I snagged.
There are several on Ebay now, but I did not see an orange one, though they seem to come up a lot.
They only difference is the small crescent shaped notch on the top back of the film prop.
Based on the pics I can also see which end to have on the prop and which end to cut off.

A few observations about this pic; again we see the C C logo, we can see there are little pads between the wooden body of the buddy and each tool, I suspect made out of cork in furtherance of friction to keep the tools/instruments in place, and lastly a little spiders nest in the crevice.

The toothbrush looks to be much more modified than I had previously noticed.
The clear tubing seems to be for a gag, but doesn’t attach to anything inside the buddy. I think it’s purpose is to get a grip on the toothbrush with your fingers so you can pull out the toothbrush, but look like a toothpaste dispensing gag.
Likewise I think the red/orange cap and white toothpaste cap are for a gag, hose up the actors sleeve attached to the buddy at the white cap, gag paste or foam passes through the buddy, exits through hole under red/orange cap.
