I don't think the three full stripes were ever shown on TOS. But I did see it illustrated somewhere, I'm thinking maybe in the tech manual.not sure what the 3 stripes were....

Since the film, the costume seems to have been retrofitted with Captain's stripes, at least for this appearance at the TMP Director's-cut premiere
