Multipass Image Template?

I was wondering if any of you guys have high resolution images of the ID part of the multipass prop? I want to make my own (a Leeloo one and one with my own face on it) but I want it to be very accurate too. Any help would be appreciated. :D
 
I'd love that digital file as well. I have a multipass that I made a few years ago with Matt's kit, but I'd love to update it with my own picture on it printed on a clean copy that I could overlay so I don't ruin the original. Would this technically be a paper prop, since it's the image section of the physical prop?
 
After closer inspection of the replica prop ID and this fan-made image, I realized how inaccurate the latter is so I'm going to clean it up and make it much more accurate.
 
I found something that may work, but it's going to take a bit of clean-up work and figuring out the right fonts and stuff:

I wasn't going for much accuracy, I had to get the barcode in there and I just filled the rest with in-jokes in the right general shape and size, using the DVD and some photos of model kits as reference. I'm sure most people could do better than my image for accuracy; my pass was made in a rush (build & graphics) in one evening, while at the same time I was packing luggage. :lol It was just a way to get a few giggles at that conference and use up some old broken junk from my workshop.

EDIT: I dug up my image file to pick out the fonts for you. The blue text below the photos is Stencil BT. The black and red stuff along the right side is Arial Black. "MULTI PASS" is Arial Black, stretched out sideways. The red-on-blue text at the bottom is a total guess, I couldn't get a good look at it in any of my reference material so I just used Adore64 (a Commodore 64 clone font I liked the look of) and wrote out the data from my barcode in case it didn't scan. Unfortunately I don't know what the font used for my name is, I converted it to vectors before saving the file.

I no longer have my reference photos so I couldn't tell you what's close to the original and what's kludged or what the text said before I changed it all. I can at least tell you that the barcode is not there at all on the real thing. Since (as explained in the post of mine MrSinistar linked) the only reason I built this was to have something to put this barcode on, I had to add it.
