Help with a paper membership card

Sorry for my other post, it looks way out of place now.

I'm trying to recreate a paper membership card from a long gone retailer.

Don't shout out the name if you know it ;)

Avery products does not carry the printable, punch out card, but I swear they used to. Any other suggestions where to look? I want those perforated "chads"

Here is the card. Maybe there is a perforating wheel at Michaels?

I have a Rotary Trimmer, a cutting board. It has a wheel blade. I got it at an office supply store, Staples I think. There are a few different brands out there. The wheel can be removed and changed for other wheel type blades. You can cut designs like the edge on victorian pictures, or score an edge for easy folding. It also has a perforation wheel. I used it on the larger square forms here:

You should be able to see it on the right and left sides of top one. This should give you the desired effect.

Yes! Excellent thank you. One follow up question, the corners on the card, I know there are corner cutters, is that a .25" angle cut? Any suggestions to get that effect?

You are going to have to figure that out and find what is available to you in your area. Craft stores and office supply stores are excellent places to find supplies. Also look online or on Ebay for specific items.

This is the corners i use most. I don't know the size, just the look . I know there is a smaller one out there.
