What was the name on the score of the music playing just after Han finds Luke and the speeders are looking for them? You hear it again when they escape from Bespin after picking Luke up.

Correct. Although, interestingly, it was scored differently to begin with.

I believe that it's lumped into 1 track with other music on the soundtrack...

No,

It is lumped in with "Rescue from Cloud City/Hyperspace".

"Snowspeeders Take Flight" is "Luke's Rescue" from the youtube link above.

Hyperspace used to be available as a standalone track on both the original RSO CD and the 1993 box set, but then the RCA/Victor release of 1997 merged it in with the Cloud City material. A bit frustrating.
