That's the only scene i know about, where you can see the whole scalpel. I am not sure about the handle.

The one above is way too small. I bought it and it is not right. Sharp as hell though.

I think the one used on the show is a custom prop. I remember Lithgow saying in an interview that the blood comes out of the scalpel when Dexter cut his cheek.

Yep, that's standard for prop knives when they're supposed to "draw blood". There are a number of tiny "channels" in the blade and handle, and a tube is attached to the end of the handle (usually run through the actor's sleeve and down his/her back so it won't be seen). At the other end of the tube is a squeeze bulb filled with stage blood. When the actor touches the knife's blade to the skin, someone off-camera squeezes the bulb and the stage blood flows through the tube, through the knife, and onto the skin. The blade itself is dull so that it won't actually cut the skin of the "victim".
