That is the mock up mask that was used in a deleted scene from ROTS, where a droid brings it to show the emperoro, and Palpatine says "No, that mask will NEVER do for Lord Vader.. The paper will chafe his burns, you mechanical imbecile.."

It's screen accurate.

It's only the perspective of the camera lense that's throwing it off a little.
