I can give you 2 screengrabs I have in my folder from doing the crossroads boxes now, I don't know if you actually need these ones but I already have the screenshots so I might as well:

U.S. Wildlife Service (only see Dean's, but Sam's name was Agent Hamil and since Dean's used Dean as his first name, it's a reasonable assumption Sam's ID would say Sam Hamil) put into Dean's crossroads box at the end of season 2. The IDs were originally used in S01E03 Dead in the Water but there's no clear shot of them in that episode.
The photo of Dean is too blurry to be useful but it is an ID from season 1, so if I were recreating it I'd try to use a screenshot or promo pic from season 1 of Dean and Sam for them, cause they're younger and their hair is different.

And the ID Sam puts into a crossroads box S03E05 when he's trying to get Dean out of his deal, Cumberland County Sheriff. Sam's alias is Jimmy Paige, Dean's is Robert Plant according to Aliases I Season Three - Supernatural Wiki
I can go through that and get a shot of Dean's ID too if you want it.

Neither were on your list so I don't know if you wanted those or not. If you want, I can use the alias list on the website I linked and go through the episodes they mention for shots of any IDs used, although I'd hate to be redundant and cover ones you've already done.

Yes if I cut the pages out we have them and are just making templates from them I will post the ones I cannot read that are not in the book tomorrow

with the help of Pixels, I was able to get a good look at Sam's health inspector ID and decipher the name on the card. Replicating their pictures on the IDs is near impossible since I can't find the same head shots used, so I made my own.

EDIT: I've removed the IDs because I'm making minor changes to them and I don't want to take over scornell's thread posting IDs. I'll make a thread when I've got something to post.

Well, they're exactly the same since those are the ones the Teddy Bear Doctors use

I guess they just need to be in a black flippy wallet thingy.

Those are the ones they show the little girl and say they are teddy bear doctors. Now, if you want them to physically say Teddy Bear Doctor at the top, that's an easy change. I'll post it after breakfast.

For the Minnesota Law Enforcement ID, it probably contains stuff like in the boxes below, except instead of DOB it's probably "EXPIRES ON"
