Rebel ‘rank plaque’, ‘rank insignia’, ‘communications badge’, or ‘command insignia’ in SW classic movies ... which pieces were used?

These are not ranks. They are more like identification/security tags. I don't think they ever been established what the purpose is just a very SWU greebie to enhance costumes. I believe that one belongs to Andor. I believe the general consensus is that it is made made from a mold that was created from a master of various doo-dads. Some may be model parts and some may be just other found parts like maybe a bearing.
Here is someone's pretty interesting take on it...

For the ESB ID badges a small section of a V8 Motor Kit Part (Battery cover) was used (painted silver with black details) for the center of the badge. Four square and two round, furniture bumps were used, along with the left over material (rubber) for the three "holes" near the bottom. I also used two pieces of skewer sticks because they were the correct diameter, but they might have used some thick wire or left over sprues from the V8 Motor Kit.Sorry this is a pet peeve of mine..

I did some quick online research and discovered that this one is Cassian Andors badge from Rogue one. The cylinder part looks like a piece of diamond, lightly knurled aluminum rod (cut in half). On the corner is what looks like a grommet or button. If not, it's an easy enough to machine out of aluminum. Below that are probably a couple of aluminum rods that were simply sliced off. The "grill" part could be from a tank model, similar to the model parts used on the original Y-Wings.
You could try posting the photo over at the Studio Modeler's section. One of the researchers there may know what it is.
More than likely, the entire badge it was molded and then cast in resin. Like the badges used in the original trilogy, it was probably spray painted aluminum and then detailed with Testors paints. This one looks like it was also weathered, probably with some black paint and thinner.
Here's a reference photo of Cassian. The jacket reminds me a little of the Luke jacket from ESB:

I’m no expert on cartridge caliber sizing so I defer to you but definitely looks like a firearm shell.
Also the badge next to it looks to be the same design used for other characters like Leia, Lando, Madine in Return of the Jedi. This design was replicated for Ahsoka and we discussed the parts here if you are interested in that one… New Republic / Rebel Alliance Collar Device Representation and other insignia.
