Questionnaire Binder from Severance S1E1

I'm brand new to the RPF so forgive me if this has already been covered (I just can't seem to find it anywhere), but I was curious if anyone had any information about the binder with the questionnaire used by Mark to ask Helly the questions in episode 1.

I started re-watching the show again recently and that prop really stood out to me as something that I would like to try my hand at re-creating.

Hello! I've actually made this before, but yes, it was never posted on RPF

Here's a link to it, and this was completely made from scratch with screenshots of the episode
so bare in mind that it might not be 100% accurate but tried my best!

I was able to get in touch with the prop master and ask her a few questions about the binder. I'll have to drop that here soon as well.
