There's 2 high-quality production studio versions I know of. Mikey from MIB 1: and Namtar from an episode of Farscape in season 1: Non-professionals have tried it with varying results. These guys have a great looking setup but don't sell their stuff: http://www.lordsofthehoof.com/index_files/construction.html This guy's tried it but didn't get far: http://www.angelfire.com/d20/roll_d3_for_this/ Other people fake it by using those german "Powerskip" stilts that look like leafsprings.
