Great idea! I'd like to see it on a thin sheet of plexi, frosted while keeping the printed piece masked off (clear). HOW you would do it, no clue.
It's so much easier armchairing a design so long as I don't have to do it. :lol

For details :

I've used an plastic card.
This card type used for make badge.

For the support I used a connector for HDD.
I removed wires and recovered the "lock wires" to stick it upside down to use its slot.

In the end I have a cut card printed on both sides stuck in an old connector.
