As in make a new pattern yourself? You would need to draw one up in Photoshop or Illustrator.

I believe people ask Kenlandrum for permission to use on of his free files and remove all of the details as a starting base. Otherwise I believe 4Neo Designs sells a cheap muscle base pattern you can use to build off.
