The IDs weren't displayed properly (Dean flashes his to the man they're talking to, Sam doesn't even produce one) in the episode they were originally used (season 1 episode 3), you only see Dean's in such detail because it's put into the crossroads box at the end of season 2, so the numbers on Sam's will probably have to be made up if it's not in a supernatural fan book.
But the name on Sam's should be Agent Sam Hamill. They introduce themselves as Agent Ford and Agent Hamill - star wars reference. Since Dean's ID says Agent Dean Ford, it's logical to assume that Sam's will also keep his first name and be Agent Sam Hamill.

I'd suggest probably keeping the codes/numbers almost the same and changing the last 2-3 digits, as if they were issued at almost the same time.
 
Ok so now on to the things withing the IDs that I don't know or cant read:

1. California Drivers - I only have Sams, it doesn't list a name, Also what info for a Dean version?

From Dean's police records & the wiki I can tell you that Sam is supposed to be 6'4" and Dean 6'1". From his police record Dean Winchester's birth date should be January 24th 1979 and his weight 175lbs but the police record is generated much later than his driver's license would be, so the weight may not be the same. Dean's eyes are green.
Sam's D.O.B. is May 2, 1983 and his eyes are brown.
If their drivers' licenses are supposed to actually be them and their info, then their real info should work.
I've never seen an American drivers' license so I have no idea what information is normally on them.

But er, in that picture Sam's sex seems to say F, for female.

You can see Dean's warrant number is 235 and the expiry date is actually 05-08-2006.
I took the screenshot from earlier in S03E05 Bedtime Stories. Dean flashes his ID clearly, then later we get the close-up of Sam's used in his crossroads box near the end of the episode.
Comparing the shot of Dean's to this one I took of Sam's ID at the end of the episode makes me think the expiry date on Sam's ID is actually also 05-08-2006.

However, since season 3 aired in 2007-2008, perhaps the year on the expiry dates is 2008 (even though it looks like a 6 to me in Dean's, especially compared to the 08) or perhaps they don't care. I'm not sure if the show is set in the same year it's filmed.

The shots of them from the episode aren't as good as the ones from the official webpage collected here:

The text is still unreadable but you can see the number under Dean's photo is 367596 although this number is shorter than the one in the picture you have. The one you have looks like it's the one used in the episode.

Sam's number looks to me like 324576190 or 334576120.
On the ID you posted Dean's number looks like 342598196 to me.

No Idea what the information at the bottom says.
I couldn't see these even being used in the episode (S01E04). Sam and Dean use the homeland security IDs to infiltrate the NTSB hangar in this episode. The staff wear portrait-oriented passes on lanyards but there's no close-ups of those either.

I have all of the info for Dean and I have one done just need Name/Number/Pic for Sam
In the episode they pose as working for this alarm company, they don't even show IDs or introduce themselves with full names. They both wear uniforms with the PF shield logo and the first names Sam and Dean stitched into the uniform, but don't give surnames or show any IDs.

Dean's eyes say brown, even though Dean's eyes are green, and his hair says grey, when it's actually obviously brown. Dean's height is actually 6'1". The information on the pass is wrong but I'm not sure what episode it's from, it seems like a lot of the basic info on the passes from the website and books are totally made up - like making Sam female on his drivers' license.

The name on Dean's ID, Dan Hermansen, is the art director for 4 Supernatural episodes. I have no idea what episode, if any, the ID is shown in, so haven't been able to find any info for it or Sam's, or find out why the info on Dean's is so obviously wrong.

This is getting annoying ._.

Very strage...in this picture the Minnesota Law Enforcement ID is blue and it looks like a different picture too.

You're right, yeah. In the episode it's used S01E15 The Benders, the ID Dean uses has a blue border at the top, not red.

Sorry for the low-res picture, I wasn't able to get the blu-ray for this one.

The badge number should be 1839 not 183 as the red-ID shows, because it shows up when the police officer runs a check on the ID that Dean shows her, being flagged as stolen.

It's hard to trust the pictures in the coffee table book or the ones posted on the wiki. All the ones we see appear altered. For example the emblem on the left side of the health inspector ID is different on screen than it appears in the book, as well as the signature being different and the name missing altogether.

I made this for my own crossroads box (work in progress) if it is helpful to anyone. The photo is blurred a bit as this more closely resembles the on screen prop ID.

I know this was forever ago but my graphics guy is almost done remaking all of the Ids that appear on screen, he will be placing them there in a little while maybe a month. I will advertise later.

You guys should submit your designs! I've seen tons of awesome FBI card designs. I'd love to buy through that website, but I haven't designed any cards myself so I can't submit them. They pay you for them too!

So it seems like the site will not accept our IDs as they are not fantasy enough and look to real. We really do not want to sell them and have to make them and ship them etc, I will keep finding a way to get them out to you guys, he really doesnt want to put in the hours and hours to duplicate them all if we can get him paid lol.
