I never thought it would be so hard to find reference for this label without just looking at film's screencaps does anyone have a reference for the label?

WARNING: side effects may include itching, oozing, weeping. Not intended for heart patients or those with nervous disorders. To make the effects of the potion permanent the drinker must obtain his or her true love's first kiss by midnight.
