Looking for TOS Klingon uniform recognition manual page

I forget who published it. Maybe it was an earlier Shane Johnson book but there was a page with the original Klingon uniform front, back, and maybe side views. The book also included some Klingon dress uniform that was totally made up by the author/artist of the book. I only mention the bogus dress uniform as a memory jog, not concerned with it. The layout was sort of like the style of Franz Joseph but maybe not as well drawn.

Does anyone have that book and if so can you scan the TOS Klingon uniform page please? Thanks.
