Back on topic, yes it´s Haydens face-cast with the half of a OT Vader helmet.
Afaik the prop dept. cut up a helmet and made identical, mirrored parts to build the new, now symmetrical ROTS Vader lid. You can see pics of the construction on the bonus DVD of the OT-DVD box.
