Gap between chestbox and armour on supreme Vader?

How do I fix it?

Yeah, to get it right you can't attach it using the pre-made slits in the suit. I know in ROTS the box didn't have straps like the OT but to put it in the right spot you've either got to add OT style straps or modify the suit to fit it in the correct spot.

The problem is patching the shirt piece so you don't show show new lines where that patch is.
 
It would be impossible to fix the factory slits and the fix not be visible.

You could look look for a matching leather material and sew a patch to exactly fit the slit openings. You might also see if you can take material from someplace else on the suit (or get another suit for material) and transplant it on the front to hide the slits.

How about fixing the chest armor?

For the chest box, just add straps. Black leather, 1 inch thick. You can use black leather belts and fix them together if you've never worked with leather blanks before.

For the shoulder armor I'd check out Danakins Rubies Vader build thread if it's still active.

James is correct, however that does not change the length of the armor. The slits for the chest box straps are placed far too low on the Rubie's suit and unfortunately there's not much that can be done. You can cut new slits higher up on the chest and sew the original ones shut but they will still be visible. The only true solution would be to get a new undersuit and cut the new holes higher up where they need to be.
