I think Phillipes gave away his remaining stock to someone on the Aliens board a few years ago.
They weren't selling, so he just gave them away.

Here's one that I redrew a while back if you can apply it to some clear acrylic:

It prints out at around 43 X 94 mm. I don't have a good screen reference shot, so I don't know if that's close to the right size and the fonts are best guess.
