When I was revisiting some of the reference images that ECL compiled to see if he had any that I missed, I came across this one and immediately thought of you. It looks like this one is sewn into Miss Piggy's glove... but not sure if it's an original or replica. To me it looks a little too new and crisp

Since I didn't get it, I'm not sure what the origin is. A google image search ended in a 404 error on what might have been the original website.

Moviefreak You probably already found the answer to this but I just randomly searched muppet on prop store and found a Fraggle that has that tag sewn into it. It looks like they attached it to the bottom of the neck sleeve. There's also another muppet on their site that shows a hand written one attached to the inside of the puppet fur
