Any ideas on how to make something like this safe to wear?

It could be easily 3D modelled and printed, getting electronics small enough to be multi function in terms of the power up cycle and then slowly fading etc would be tricky at such a small size.
Having it just basically light up would be possible with a watch battery and a white SMT or 1.8mm wouldn't be too tricky.

It's an entire costume in a button. I don't know if the contact lens is optional or not.

Implant a piece of ferrous metal under your skin. A magnet in the button. Voila.
