Harry Potter R.A.B. Note Help

I'm a big Harry Potter fan and would like to make a replica of the R.A.B. note Harry finds in the locket at the end of Half-Blood Prince.

I was hoping someone with either the Noble Collection replica or insight to the actual prop could help me with the proper dimensions of the note and especially how to fold it. I spent an hour and a half with a piece of paper today trying to fold it so it would match the screen shots I have, but just couldn't figure it out.
