Custom Plaque Help

I am looking to make if possible, outsource if not, a plaque similar in nature to the bronze plaques seen in localized establishments.

I really like Jmtwo's here:

But I am not looking for an Xavier's School plaque (but one would be cool to have).

Any help would be appreciated.

You could get everything you need at a hardware store, buy some wood to cut out for the sign and the border, and some plastic letters used for signs/house addresses. Glue together and then apply some metallic paints.
