Here is an additional contribution to the cause. After some more research I created a new contact lens label based upon the accurate content of the original and tried to match the exact fonts / graphics. It should be printed 60 mm x 32 mm. Not knowing who can support which files, I uploaded both a BMP file and an AI file.
I understand these are widely available and fairly inexpensive, however I was able to obtain a small lot of the Alcon Opti-Free contact lens cases and have extras if anyone is interested for just the cost of postage.
For those having difficulties sourcing the Uniball pen, they do have a current product, Uniball Vision Exact Rollerball, which is the exact same case and design, it just does not have the label on the reverse which reads Mitsubishi Pencil with the barcode.
I would still like to obtain an actual Uniball Eye Needlepoint in black if anyone has a lead on one.

I had an old Wenger sitting around that I had since I was a kid, it looked very similar. Dug it out of the bottom of a draw and took some pics to post when I noticed it wasn't quite right. I figured the actual one was a cheap knockoff of this model so did a Google image search for '4 tool pocket knife' the image happed to show up as it was for a listing of 4 knives. So I guess it was a bit of luck as well.
