I made a scan of the document in two pieces, problem is, I don’t know how to stitch them back together.
Off course, this is the aged (66 year old) version. If you want a screen accurate one you need to clean this one up.

The size of the document is, 33 X 20 cm.

I've got this so far. I'm not happy with the font. It's a free one called Underwood Champion, and the 4s look okay, but the center stroke of the capital W doesn't reach high enough. Ah well, what does the collective think?
EDIT - There's another font called DS Corona Cyr which has better-looking letters, but it isn't monospaced so I'd have to play around with kerning.
