Batman Begins - Joker card evidence labels?

OK, help me out here!

A while ago a fellow member made me a image of the evidence labels that I could print out.

I had a PC crash a few months ago and now it turns out I have lost this file (I could have sworn I had everything backed up)

I cannot for the life of me remember who made it for me though, it was so long ago.

Can anyone help me out and make a new one?

For some reason couldn't get image tag working.

Looks like you have a few options Tony, but I was the supplier of the Card and Tag originally. Let me know if you want me to resend it.
